I. Identification

A. Specifics (Questions including name of respondent, line # of respondent, sex of respondent, most convenient phone number to reach respondent, whose phone number it is, official BAAC Group number, whether respondent is group leader, name of group leader, line # of group leader, sex of group leader, most convenient phone number to reach group leader, who phone number it is, where the branch office of BAAC that this group goes to is, and name of BAAC officer that this group talks to most often)

II. Group Origin (Questions including when group was established, reason for forming group, from where idea came, main benefits of being a member of a BAAC group, and what training group members have received)

III. Characteristics of Group Members, Membership Procedures, & Meetings (Questions including list of members, each member?s information [person?s primary occupation, how many rais person owns, highest level of schooling completed, whether person has a relative in BAAC group, where person lives, whether person has been in group since beginning, how many years person has been a member], how were original members determined, how someone can become a member today, whether the process has changed, how many member have joined in last five years, why these people didn?t join when group was started, whether anyone has been refused membership, why, whether there are people who would like to be members but cannot, and why)

A. People who have left the group (Questions including whether anyone who was originally part of the group is no longer, names of these people, information on ex-members (person?s primary occupation, what his/her primary occupation was when he/she was a member, how many rais person owns, highest level of schooling completed, whether person has a close relative in BAAC group, how many years person has been a member of group], why these people left group, and the circumstances)

B. Meetings (Questions including how many meetings group has had in past 12 months, subject of meetings, who is supposed to attend, whether there is a penalty/fee if a member misses a meeting, the penalty/fee, how often has it been applied over past 12 months, and whether there is someone who takes notes & records details of the meetings)                                    ?

IV. Group Leader and Group Communication (Questions including how did respondent become group leader, when, and most important qualifications for a group leader)

A. Communication between Group Leader and other Members (Questions including in a typical week how many group members does respondent talk to less than once a week, once a week, 2 or 3 times, 4 or 5 times, or more than 5 times, whether group member seek or receives advice on which crops to grow, on pesticide or fertilizer usage, production advice, or raising livestock, who has final decision on what crops to grow, on which pesticides and fertilizers to use, & on production techniques, whether group leader is aware of the quality of other members?work, whether other group members are aware, whether group leader is aware of the debt & savings of group members, whether other group members are aware, whether group leader is aware of debt group members have to individuals, whether other group members are aware, whether group leader is aware of assets of other members, whether other group members are aware, whether respondent thinks BAAC is aware of assets of group members, whether group leader ever tried to deter members from borrowing money from other sources, whether other members have tried to deter, whether the respondent thinks that BAAC keeps track of debt & savings members have with other institutions, whether respondent thinks BAAC keeps tracks of debt members have to individuals, whether respondent think BAAC has tried to deter members from borrowing from other sources, and whether respondent thinks BAAC has tried to deter members from saving with other sources)

D. Savings (Questions including whether any members have savings at non-BAAC institutions, how many, with what institutions [Commercial Bank, PCG, Rice Bank, Agricultural Cooperative, or Other], and main reasons for saving at these institutions instead of BAAC)

E. BAAC Inspections (Questions including whether this BAAC group has ever been inspected by BAAC officers, how many times BAAC group has been inspected, when inspection(s) occurred, who was inspected, in past 12 months how many times did a BAAC officer come to village, in past 12 months how many times did group leader visit BAAC branch office, during past 12 months how many times did a typical group member visit BAAC branch office, whether group leader has a responsibility to tell the BAAC if a member is likely to need to postpone repayment of a loan, whether group leader communicates with BAAC when a member has a problem with repaying loan, and whether this communication has helped the BAAC correctly evaluate borrower?s circumstances & come up with a good plan for repayment)

V. F. Collateral Taken (Questions including how many times BAAC has taken members collateral as penalty for default, from whom, why did they default, how this affected group as a whole, whether respondent knows of cases of BAAC taking collateral, and his/her description of such cases)

G. Group Members Repaying BAAC Loans for Other Group Members (Questions including how many times have group members repaid loans for other members, why assistance was given, why they were unable to repay, whether group funds were used to repay, relationship between payer and loanee, when assistance will be repaid, whether assistance was repaid with interest, and how this affected group as a whole)

H. Group Members Repaying Non-BAAC Loans for Other Group Members (Questions including how many times group member repaid loans for other members, why assistance was given, why they were unable to repay, whether group funds were used to repay, relationship between payer and loanee, when assistance will be repaid, whether assistance was repaid with interest, and how this affected group as a whole)

I. Borrowing without First Response (Questions including how many times BAAC has let a member borrow more even though they or another group member had already borrowed and not yet repaid loan, who borrowed again, why were they unable to repay original loan, and why the BAAC allowed them to borrow again)

J. Borrowing from PCG to pay BAAC Loans (Questions including how many members are members of a PCG, whether any member has ever borrowed from PCG to repay BAAC loan, when, why borrower had to borrow from PCG to repay BAAC, and type of BAAC loan)

K. Borrowing from another institution to repay BAAC loans (Questions including whether any group member has ever borrowed from some other institution [Commercial Bank, Agricultural Cooperative, Rice Bank, Gov?t Savings Bank, or other institution] in order to repay a BAAC loan, when, why borrower had to borrow from other institution to repay BAAC, and type of BAAC loan)

L. Borrowing from Moneylender to repay BAAC loan (Questions including whether any group member has ever borrowed from a moneylender in order to repay a BAAC loan, when, why borrower had to borrow from moneylender to repay BAAC, and type of BAAC loan)
